2. Application form and student’s declaration, handsigned.
3. High Secondary School Certificate with mark sheet (in English or translated into Polish by sworn translator), with recognition from NAWA (if required).
4. Apostile or legalization of High Secondary School Certificate and mark sheet to HSS certificate
5. Passport size photo.
6. Health insurance policy – valid for one year; 30 000 euro cover..
7. Certificate of English proficiency ( listed here https://pb.edu.pl/studyatbut/wp-content/uploads/sites/114/2025/08/Accepted-English-Language-Certificates.pdf )
